Refer a Patient

Thank you for entrusting Cancer Care Northwest with your patient’s oncology care. We are committed to providing exceptional, comprehensive treatment. To submit a referral, you have two options:

• Epic System Users, please send your referrals electronically to CCNW. Locate our clinic easily within Epic by searching for “CCNW”. View our Epic referral guide for more information. 

• All other referrals, please complete the CCNW patient referral form prior to faxing medical records, please contact a scheduler at any of our offices. This ensures that we receive all necessary documentation and delivery of records. To reach a scheduler, call one of our offices and ask to speak with a scheduler.
