Cancer Care Northwest is required by law to maintain the privacy of your health information, to provide you with a notice of our legal duties and privacy practices, and to follow the information practices that are described in the Notice of Privacy Practices.
You have the right to request a copy of your health information. You have the right to ask for a copy of your health information in the format you prefer. If CCNW cannot provide it in that exact format, they will work with you to choose another readable format that works for you. You have the right to request that your health information be sent to a person or entity of your choosing.
Obtain your medical records via MyChart
MyChart, powered by Epic, is a free service that offers you online access to your health record. Whether you are at work, at home or on the go, you'll have secure access to view visit summaries and care instructions, test and imaging results and direct communication with your CCNW care team via MyChart. You can also view billing information and pay your bill through MyChart.

To request a copy of your medical records to be sent to you or someone else, please complete the Authorization for Disclosure form. All information is required for completion except the sensitive information section which is optional.

Processing time
CCNW follows Washington State Law which allows for up to 15 business days to respond to a written request; however, CCNW will process your requests as soon as soon as we can.
Requesting records for someone else
If you are an attorney, contracted third-party or a state/federal agent requesting records on behalf of someone else, please fax your request to 509-252-9316.
