Cancer Care Northwest respects the health information rights for our patients and has partnered with Provider1st to securely provide medical records and x-ray imaging copies to patients, guardian/legal representatives or 3rd party requestors with HIPAA authorization.  

If you have any questions or would like to check the status of your request, you can reach Provider1st by phone at: 855-514-2378. 

If you are a patient and would like your medical records instantaneously, please log into or request access to MyChart patient portal and download your records.
