Your Patient Rights

Respecting your privacy and rights as a patient are of high importance to our health care teams. If you have questions about Cancer Care Northwest’s Privacy Notice, or if you believe your privacy rights have been violated, please contact the Compliance Officer at (509) 228-1013. 

As a patient, you have rights pertaining to your care. They include the following: 

  • Right to inspect and copy information. Right to request to amend or correct information.
  • Right to an Accounting of Disclosures.
  • Right to request restriction on release of information
  • Right to request confidential communications
  • Right to ask Cancer Care Northwest to review a denial of access to your health information.
  • Right to file a complaint with Cancer Care Northwest or the U.S. Secretary of Health and Human Services.
  • Right to a paper copy of this Notice. To revoke any previous authorization or request.
  • Right to request that health information be provided to you by another means or at another location. Please sign, date and give us your request in writing.
  • Right to restrict disclosure of health information to your health plan for services you have personally paid for.


The following requests to Cancer Care Northwest must be made in writing (forms available in the Health Information Management department):

  • Psychotherapy Notes: if we record or maintain psychotherapy notes, we must obtain your authorization for most uses and disclosures of psychotherapy notes.
  • Marketing Communications: we must obtain your authorization to use or disclose your health information for marketing purposes other than for face to face communications with you, promotional gifts of nominal value, and communications with you related to currently prescribed drugs, such as refill reminders.
  • Sale of Health Information: disclosures that constitute a sale of your health information require your authorization. 


Other uses and disclosures of your health information that are not described in this Notice will be made only with your written authorization.  You have the right to cancel prior authorizations for these uses and disclosures of your health information by giving us a written revocation. Your revocation does not affect information that has already been releases.

If you have questions about Cancer Care Northwest’s Privacy Notice, or if you believe your privacy rights have been violated, please contact the Complaince Officer at 509-228-1013. You will not be retaliated against or penalized for filing a complaint.

If you believe your personal and/or financial information has been breached, you can place a fraud alert on your credit report.  This can help prevent an identity thief from opening additional accounts in your name.  As soon as the credit bureau confirms your fraud alert, the two other credit bureaus will automatically be notified to place alerts on your credit report and all three reports will be sent to you free of charge.
